PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Shreveport, Louisiana.

Schedule & Work Details

• Monday through Friday, 8:00 AM to 4:30 PM
• On-call: One on call Saturday and Sunday rotation per month
• Coverage areas include 50 mile radius around Shreveport, LA
• Mileage reimbursed between patient visits
• Hourly pay rate: $48-53.00 per hour

Key Responsibilities

• RN field case manager- will be responsible for day-to-day visits and symptom management, supply ordering, coordination of medications being refilled using electronic system, documentation of visit appropriate for hospice and synching them back same day.
• IDG notes and attendance every other week.
• Communication with the office and physicians, writing verbal orders, and communicating with agency supervisory staff.

Required Qualifications

• Current Registered Nurse license
• Home health nursing experience required
• Hospice experience and HCHB experience
