As part of Guardian Angels' continued growth, this is a newly created opportunity as we transition from a contracted therapy model to a program supported by Guardian Angels. Position Summary:
The Director of Rehabilitation is responsible for the complete management of day-to-day operations. Responsible for supervising therapy personnel, oversees recruitment and interviewing, provides clinical education and training, implements clinical programs, ensures compliance with regulatory and company policies and procedures, maintains financial accountability, manages appropriate staffing levels to promote facility efficiency, provides customer service, attends facility meetings, implements employee retention strategies, and provides direct patient care within one’s discipline. The Director of Rehabilitation will build a culture of teamwork and engagement to provide exceptional care, customer service, and support for residents/clients and their families. Services are provided in accordance with the mission of Guardian Angels.
Job Duties:
- Create and maintain an environment that is supportive, advocates for the residents, and provides high-quality care that meets all the needs of the residents/clients in our care, including physical, mental, emotional, social, and spiritual.
-Maintains a positive work atmosphere by demonstrating and communicating in a professional manner to foster positive relationships with customers, residents/clients, co-workers, and managers.
-Responsible for promoting company philosophy and consistently displaying professionalism and a positive attitude conducive to a thriving culture.
-Collaborate with the other departments to provide comprehensive services.
-Builds referral sources to promote the therapy department.
-Participates in Quality Assurance meetings to support quality and safety outcomes for residents and staff.
-Attends and participates in interdisciplinary team meetings, patient/resident care conferences, utilizing review meetings, and other meetings as required or directed.
-Completes the required related documentation within the resident's/clients electronic charting software, ensuring compliance with regulations and established best practices.
-Attend and provide leadership at Care Conferences.
-Responsible for clinical discussions related to resident/client status and discharge planning.
-Provides direct resident/client care within the assigned facility as determined by scheduling, size of department, and the needs of the facility.
-Responsible for working with evaluating therapists to set appropriate treatment plans in the computer software system and schedule appropriate resident/client treatment plans to meet evaluation recommendations for all payer sources.
-Responsible for adhering to all regulatory standards as they pertain to resident/client treatment and delivery of care, and to ensure that all rehab needs are met for each patient.
-Responsible for ensuring that all documentation is completed within the minimum timeframe set by the company and that documentation accurately reflects the therapy services being provided.
-Performs chart audits monthly as directed by supervisors and develops action plans to improve documentation. Reports audit results monthly.
-Responsible for the financial operations of the rehab department and is fiscally responsible with resources. Ensure equipment and supplies are maintained and in adequate condition.
-Demonstrates the ability to understand key operational reports.
-Meets and/or exceeds efficiency standards within the department.
-Responsible for billing, invoices, and month-end duties, completion and reporting done in an accurate and timely manner.
-Establishes effective communication systems with employees, making sure staff are aware of department updates and changes. Keeps team members abreast of changes to policies and procedures, as well as other initiatives and educational priorities.
-Ensures an organized, neat, and clean department at all times to promote safety and excellent patient care/customer service.
-Responsible for the supervision and guidance of all therapy personnel.
-Performance management responsibilities include hiring, orienting, supervising, evaluating, providing feedback, and/or coaching, and taking corrective action.
-Responsible for training new staff members on the computer software system and ensuring compliance.
-Promotes self-development and growth. Looks for learning opportunities and provides education to others when needed.
-Attends in-service as required and provides training to team members as appropriate and as required by regulatory standards.
-Holds team members accountable for completion of work duties, maintaining quality standards, providing excellent customer service, and the completion of required training in a timely manner.
-Participates in facility and department celebrations.
-Fosters a teamwork environment, ensuring all team members do their part.
-Responsible for timely and accurate review and approval of time logs before submitting for payroll.
-Responsible for minimizing the use of overtime by strategizing with the rehab team and Administration, as well as for receiving approval for overtime prior to the occurrence.
-Responsible for ensuring adequate staffing levels and minimizing the use of per-diem and contract therapy hours by scheduling and strategically planning.
-Complies with all Human Resources policies and policies set forth by the organization.
-To assist in providing a dignified, respectful, resident-centered environment for residents.
-Follows universal precautions, facility infection control guidelines, sanitation, and safety standards of practice in all work and activities.
-All duties will be performed in accordance with current standards, guidelines, state and federal regulations.
-Uphold confidential information as required by Guardian Angels policy and procedure and as related to HIPAA.
-Uphold the mission, vision, and values of Guardian Angels.
-Performs all other duties as assigned.
Requirements
Education/Experience:
-Bachelor of Science in Physical Therapy, Occupational Therapy or Speech Language Pathologist (Master's or Doctorate as required per graduation date of employee) from an accredited program is highly desired.
-Successful completion of the National Certification Examination and licensed in the state of Minnesota.
Experience:
-Minimum of five years of prior experience in a rehabilitation setting, such as acute care, inpatient rehab, outpatient clinics, SNFS, long-term care, assisted living, memory care, or home health.
-One to two years’ supervisory experience is preferred.
-Familiarity with fall prevention strategies and mobility training for seniors.
-The ability to use therapeutic techniques to help clients build or restore skills.
-Interdisciplinary management experience and the ability to make decisions.
-Proven leadership and program development/supervisory experience preferred.
-Maintains working knowledge of program implementation for Medicare and other payer sources.
-Ability to lead a team and motivate them to be their very best.
-Strong problem-solving and critical thinking skills are essential.
-Excellent organizational skills.
-Ability to multitask, prioritize, and work independently.
-Excellent verbal and written (read, write) communication skills.
-Proficient with Microsoft Office 365 required. Net Health software experience required. Familiarity with other healthcare software, such as PointClickCare (PCC), is a plus.
-Maintains good attendance and punctuality.
-Ability to work flexible hours and be on call.
-Ability to travel to other locations as needed.
