Optima Health provides Mobile Primary Care to Senior Care Facilities and patients across Southern California. We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for the Redlands, Yucaipa, Loma Linda, Mentone, Banning, and San Bernardino regions.

Responsibilities

• Deliver patient-centered care in Senior Living Facilities
• Conduct assessments, order diagnostics, prescribe, and manage treatment plans
• Collaborate with supervising physicians and interdisciplinary teams
• Ensure compliance with state and federal healthcare standards

Requirements

• Active California NP or PA license
• 1–2 years in geriatrics, internal medicine, or primary care preferrred
• Ability to work independently in a mobile/field environment
• Strong interpersonal and communication skills
