Description

We are seeking a dedicated and compassionate Nurse Navigator to join our healthcare team. The ideal candidate will possess extensive clinical expertise and a strong commitment to patient-centered care, guiding individuals through medical journeys with professionalism and empathy. The Nurse Navigator provides assessment and coordinates supportive services for residents transitioning between levels of care at Carolina Meadows and/or external health facilities or acute care settings. Additionally, this position is responsible for providing supportive follow up as necessary following Community Response events from residents in Independent Living, including evaluating the need for additional supportive services that will allow residents to reside safely within the Carolina Meadows community. The Nurse Navigator serves as a guide to connect residents with the community resources necessary to meet their dynamic needs. The position will also support the Carolina Meadows Medical Practice with additional clinical support on an as needed basis.

Requirements

Graduate of an accredited school of nursing.
Two (2) to four (4) years of long term care with geriatric medical services and transdisciplinary team experience.
Current licensure as a licensed practical nurse or registered nurse in North Carolina.
Current CPR certification.
