Job Summary The Nurse Practitioner Inpatient Transplant (Non-Operative) is a licensed, certified, non-physician healthcare provider who delivers direct patient care to transplant patients across the continuum of inpatient care. This role has delegated prescriptive authority and is credentialed and privileged as an independent provider, recognized by Medicare and Medicaid. The Advanced Practice Provider delivers care in accordance with the scope of practice established by the supervising physician. This role focuses on the management of pre- and post-transplant patients in the inpatient setting, supporting transplant programs such as kidney, liver, and other solid organ transplants. The provider collaborates closely with transplant surgeons, hepatologists, nephrologists, and multidisciplinary teams to ensure optimal patient outcomes. Essential Functions of the Role * Performs comprehensive and focused assessments of pre- and post-transplant patients, including detailed medical histories and physical examinations. * Monitors and manages complex transplant patients, including immunosuppression therapy, organ rejection surveillance, and post-operative complications. * Orders, collects, interprets, and evaluates laboratory and diagnostic tests, including transplant-specific monitoring. * Prescribes and manages medications, including immunosuppressive therapies, ensuring adherence to transplant protocols and safety standards. * Develops and implements evidence-based, individualized care plans in collaboration with transplant physicians and multidisciplinary teams. * Provides daily inpatient management, including hospital rounding, coordination of care, and discharge planning. * Identifies early signs of complications such as infection, rejection, or organ dysfunction and initiates appropriate interventions. * Educates patients and families on transplant processes, medication adherence, post-transplant care, and lifestyle modifications. * Coordinates care with case management, pharmacy, social work, and outpatient transplant teams to ensure continuity of care. * Maintains accurate, timely, and detailed documentation of patient care and outcomes. * Initiates consultations and referrals when patient needs exceed scope of practice or require additional specialties. * Participates in transplant call coverage and may provide care beyond regular schedules based on program needs. Key Success Factors * Advanced knowledge of transplant medicine, including organ transplantation processes, immunosuppression management, and post-transplant complications. * Strong understanding of inpatient care management for medically complex patients. * Excellent verbal and written communication skills. * Effective interdisciplinary collaboration and interpersonal skills. * Strong critical thinking and complex problem-solving abilities in high-acuity environments. * Proficiency in electronic medical records and healthcare technology. * Ability to develop and implement comprehensive, outcomes-driven care plans. * Ability to educate and counsel patients and families on complex medical regimens and long-term care requirements. * Skill in interpreting transplant-specific laboratory and diagnostic data. Qualifications * Texas RN licensure or RN licensure with compact privilege from a Nurse Licensure Compact state, along with Texas APRN authorization. * Current license to practice as a Nurse Practitioner in the state of Texas. * Basic Life Support (BLS) certification required within 30 days of hire or transfer. * Certification from an accredited board as an APRN in the applicable patient population required. * Preferred: Experience in transplant, critical care, or inpatient surgical specialties.
