At HCR Home Care, we believe exceptional care begins with exceptional people. We're seeking a compassionate and motivated Occupational Therapist (OT) to provide individualized therapy services that help patients regain strength, confidence, and independence in their own homes.

What You'll Do

• Provide skilled occupational therapy services as prescribed by the attending physician.
• Teach patients and families how to adapt daily routines for safety and independence.
• Educate on the use of adaptive equipment, prosthetics, and assistive devices.
• Assess home environmentsand develop personalized plants to prevent falls.
• Collaborate closely with nurses, therapists and other members of the care team.
• Document patient progressand communicate changes promptly to the physician.
• Participate in case conferences and contribute to a culture of clinical excellence.
• Support home health aide services as needed.

Work Environment

• Home based position providing care across assigned territories. (Hybrid - a combination of home visits to patients, documentation at home and office visits required for regularly scheduled meetings)
• Classified as medium work (occasional lifting up to 50 lbs.).
• Involves standing, walking, and kneeling during patient care.

Requirements

What You Bring

• Graduate of an accredited program of Occupational Therapy.
• Current, valid OT license issued by NYS Department of Education.
• Minimum of one year of experience as a licensed occupational therapist.
• Strong communication, teaching, and critical thinking skills.
• A genuine passion for helping others live safely and independently at home
