What You’ll Do as a Home Health Physical Therapist:
- Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
- Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
- Adjust treatment plans and techniques as needed to achieve optimal outcomes.
- Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
- Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
- Document all visits, assessments, and progress notes accurately and in a timely manner.
- Support patients in maximizing their independence and improving quality of life—wherever they call home.
Primary Function
The Physical Therapist assumes responsibility and accountability for the delivery of appropriate physical therapy services within a framework of interdisciplinary collaboration and consultation.
- Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities to attain the highest level of physical function.
- Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
- Develop/implement a conditioning/rehabilitation program consistent with the physicians' Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
Job Responsibilities
Essential Functions of Position
- Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities to attain the highest level of physical function.
- Test/screen the patient's physical strengths to assist the physical in evaluating the patient’s level of function and record findings to develop or pursue treatment programs and establish measurable training objectives.
- Develop/implement a conditioning/rehabilitation program consistent with the physicians' Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
- Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
- Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plan that reflects the current plan of care as related to therapy and supervise/evaluate staff’s performance. Monitor the appropriate completion of documentation by physical therapy assistants and home health aides as part of the supervisory/leadership responsibility.
- Report the patient’s progress to the patient’s physician, Clinical Manager, staff, patient, and family. Submit evaluation, treatment plans, progress reports, and discharge summaries to the supervisor and care management staff.
- Accurately, promptly, and thoroughly document patients’ care observations, interventions, and evaluations. Assure that interim (verbal) orders received from the physician are promptly and accurately documented, submitted for physician signature, and implemented.
- Plan and administer prescribed skilled therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical function.
- Test/screen the patients' physical strengths to assist the therapist in evaluating the patient’s level of function and records findings to develop or pursue treatment programs and establish measurable training objective.
- Performs other duties as assigned.
- Maintains current licensure certifications and meets mandatory continuing education requirements.
- Participate in the Home Health’s Quality Assessment and Performance Improvement (QAPI) program.
Requirements
About You
Qualifications – What You’ll Bring:
- Master’s degree in Physical Therapy from an accredited program.
- Current PT license in the state of employment (or eligibility to obtain).
- CPR certification required.
- Strong communication, assessment, and problem-solving skills.
- Ability to work independently in a variety of home environments.
- Valid driver’s license, auto insurance, and reliable transportation.
Preferred Experience (Not Required):
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Education/Licensure:
- Master’s degree from an accredited Physical Therapy Program (approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Physical Therapy Association)
- PT Licensure in the state of practice
- Current CPR Certification
Experience:
- One year's experience in hospital or home health is preferred.
Equipment/Tools/Work-Aids:
- Must be able to effectively operate a computer, facsimile equipment, copier, and cell phone. Must have and maintain transportation to be used for work.
Specialized Knowledge and Skills:
- Organizational and leadership abilities
- Knowledge of roles of all disciplines providing home health services
- Excellent patient assessment skills; good oral and written communication
- Thorough knowledge of managed care principles, regulatory guidelines (i.e., Medicare, Medicaid, ACHC Accrediting Organization, and human resource), management principles.

Directions Received
This position reports to the Administrator, Executive Director, or Patient Care Manager.
