Linden Ponds, an award winning Erickson Senior Living community, is seeking a dedicated Physical Therapist to support our residents in our on-campus outpatient clinic. We prefer a fulltime team member, but we are flexible and open to parttime schedules for the right candidate. Because all clients live within our vibrant 108acre community, you’ll enjoy a consistent schedule with no additional travel.

What You’ll Do
- Provide outpatient physical therapy services to older adults within our campus
- Implement individualized care plans that honor each resident’s strengths, preferences, and routines
- Partner with an interdisciplinary clinical team to ensure a coordinated, resident-first approach
- Document all care to meet regulatory and reimbursement requirements
- Support specialty programs such as urinary incontinence, vestibular rehab, home safety assessments, movement disorders, and low vision

Requirements
What we require:
Must possess a valid and current state Physical Therapy license. Degree and training must meet current Medicare and state requirements.
The ability to complete documentation to meet reimbursement and regulatory requirements.
Therapeutic experience in older adults across multiple diagnostic groups is preferred.
