Job Summary

Provides management and coordination of operations of Community Health and Adult Foster Care programs (AFC).  Develops services and supports to improve access and referral to health promotion and disease preventative programs within the Tribal and greater community. Responsible for hiring, training and supervising employees; ensuring adequate staffing of quality personnel for the AFC program.  Responsible for ensuring AFC program meets regulatory requirements. Collaborates with Health and Wellness Clinic to coordinate case management to identified clients while contributing to the accomplishment of the Tribe’s holistic health and wellness objectives for patients and families.  Supervises Community Health program staff.

Essential Functions:

  • Provides program management and staff supervision of Community Health and Adult Foster Care programs.
  • Responsible for implementing plans for community-based health programs aimed at the prevention of disease and promotion of health specific to health conditions of the community.
  • Coordinate and direct all AFC activities in accordance with policies and procedures; ensuring program is compliant with all laws, rules, and regulations.
  • Responsible for Title VI grant activities, in coordination with other departments.
  • Develops plan for contacting patients after hospital discharge or ER visit to coordinate follow-up, answer questions, review medication changes, and coordinate referrals such as physical therapy or home health care.
  • Ensures preauthorization for Contract Health Services and health insurance are completed as necessary.
  • Participate in quality assurance/quality improvement review activities as determined by Health and Wellness Leadership.
  • Conduct initial and ongoing assessments of AFC residents for program entry and continued participation; develops service plans, schedules and updates service information as indicated by resident need. Conducts (or ensures occurrence of) on-going care conferences/assessments as per policies and procedures with family members/resident representatives
  • Monitor residents' physical and mental functioning psychological well-being and nutritional status to ensure care needs are met in accordance with resident service plans. Notify physician and family and take appropriate action when significant changes in condition occur.
  • Develops annual program budgets and adheres to Tribal fiscal policies.
  • Conducts assessment of community members as needed or requested.
  • Performs a variety of other duties as assigned by supervisor.


  • Ensures the hiring, training, promoting, assigning, disciplining and discharging of employees are in accordance with Tribal Law, including without limitation, the Tribal Employment Rights Ordinance (TERO). Assures staff adherence to all policies and procedures, work, safety and administrative rules, Tribal Law and State Regulations.

Education, Experience, and Licenses:

  • Requires a current Oregon Registered Nurse (RN) license or Bachelor’s degree in public health, business management or administration, or a closely related field of study.
  • Requires a minimum of five (5) years’ experience as an RN or in community health, case management or public health and a minimum of three (3) years of supervisory experience
  • OR equivalent combination of education, training, and experience.
  • Basic Life Support required
  • Must have experience working with Community Health Workers.
  • Must maintain a valid Oregon driver’s license or obtain one upon hire, qualify for the Tribe’s vehicle insurance, and provide proof of personal vehicle insurance.

Qualifications, Registrations, and Certifications:

  • Must have a working knowledge of the Privacy Act/HIPAA and its applications.
  • Ability to work independently on assignments with technical complexity, however, most assignments are of standard complexity.
  • Native American employees preferred, knowledge of Native American culture required.
  • Expands and improves current knowledge base by attending continuing education courses as deemed necessary to meet personal development plan.
  • Able to communicate effectively; maintain an effective working relationship with clients and staff; effectively coach employees toward maximum performance; and improve current program access and efficiency
  • Ability to establish and maintain effective relationships with individuals inside and outside the organization including clients and their family members, health care providers, and other agency personnel. Requires ability to explain and discuss difficult information, establish rapport, exercise patience, and gain understanding.
  • Requires working knowledge and proficiency in use of office equipment and computer software such as Microsoft Word, Excel, Power point, and Outlook.
  • Requires excellent verbal and written communication skills, business, and telephone etiquette.
  • Must have excellent organizational skills, attention to detail and record keeping skills.
  • Must have ability to respond courteously and tactfully to complaints and client requests, and to maintain confidentiality.
  • Must understand delegation vs assignment of work in the context of nursing.
  • Knowledge of and sensitivity to Native American culture required.
