We are seeking a skilled and compassionate Nurse Practitioner to deliver high-quality primary and palliative care services to our senior residents. Located on a senior living campus known for providing exceptional, independent living, assisted, living, and memory care services. We provide the opportunity to obtain CAPC designation for Palliative Care as part of our team.
Key responsibilities:
Provide comprehensive primary care and palliative care to residents in collaboration with our interdisciplinary team Conduct routine health assessments, manage, acute and chronic conditions, and prescribed medications as needed Develop an implement individual individualized treatment care plans Provide health, education and care strategies to promote patient’s aging in place Maintain accurate and up-to-date medical records
Requirements
Qualifications:
Current Nurse Practitioner license in New York State Certification in Adult or Adult-Geriatric Preferred Experience in Primary care, Geriatrics, or Palliative care is strongly preferred Must have a minimum of 3000 hours of clinical experience to be qualified to work as an independent Nurse Practitioner Strong computer skills, ability to adapt and learn a new EMR quickly, excellent communication and interpersonal skills Commitment to compassionate, resident centered care Ability to manage multiple chronic conditions in a community-based setting to prevent acute hospitalizations or transitions to higher level of care

We understand the unique needs of older adults and individuals facing serious or life-limiting illnesses. Our practice provides care in various settings, including homes, assisted-living facilities, another long-term care environments. We believe in a patient centered approach, focusing on each individuals, physical, emotional, and social needs.
