We are looking for a dedicated therapist to provide care on a per diem basis.

The Opportunity
We are seeking a passionate and skilled Physical Therapist to join our team on a per diem basis. This is an ideal opportunity for a professional who desires a flexible schedule and the autonomy to manage their own caseload. You will be responsible for providing in-home physical therapy services with a primary focus on fall prevention, balance training, and restoring functional independence.

Responsibilities

• Provide one-on-one, in-home physical therapy to patients on an as-needed basis.
• Conduct comprehensive patient evaluations to develop personalized treatment plans.
• Implement evidence-based interventions to improve balance, strength, and mobility.
• Educate patients and their families on home exercise programs and safety.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians and other healthcare professionals to ensure coordinated care.

Requirements

Qualifications

• Master's of physical therapy degree or higher from an accredited program.
• Current and valid New York State Physical Therapy license.
• Must have their own reliable mode of transportation.
• Experience with a geriatric population or in-home care is a plus, but not required.
• Excellent communication, interpersonal, and time-management skills.
