We are looking for a dedicated therapist to provide care on a per diem basis.
The Opportunity We are seeking a passionate and skilled Physical Therapist to join our team on a per diem basis. This is an ideal opportunity for a professional who desires a flexible schedule and the autonomy to manage their own caseload. You will be responsible for providing in-home physical therapy services with a primary focus on fall prevention, balance training, and restoring functional independence.
Responsibilities
Provide one-on-one, in-home physical therapy to patients on an as-needed basis.
Conduct comprehensive patient evaluations to develop personalized treatment plans.
Implement evidence-based interventions to improve balance, strength, and mobility.
Educate patients and their families on home exercise programs and safety.
Maintain accurate and timely clinical documentation.
Collaborate with physicians and other healthcare professionals to ensure coordinated care.

Requirements
Qualifications
Master's of physical therapy degree or higher from an accredited program.
Current and valid New York State Physical Therapy license.
Must have their own reliable mode of transportation.
Experience with a geriatric population or in-home care is a plus, but not required.
Excellent communication, interpersonal, and time-management skills.
