Summary
The Advance Practice Nurse (APN) Certified Nurse Practitioner in Primary Care Service to provide a full range of services emphasizing - timeliness - and the advancement of Veterans Affairs (VA) mission. The NP provide advanced clinical practice - consultation and management.
Duties
- The NP practices to the full extent of their education - training - and certification
- Core functions are outlined in the clinical privileges that correspond with this position
- The NP has prescriptive authority for a panel of patients or program as indicated by their position
- Major duties and responsibilities of the position but not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence- based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members Prescribes pharmacologic and non-pharmacologic interventions Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

- Relocation/Recruitment Incentives: Not Authorized EDRP Authorized: This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program
- You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment
- Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application
- Former EDRP participants ineligible to apply for incentive
