PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Louisville, Kentucky. Job Description & Requirements Specialty: Home Health Discipline: RN Start Date: 06/08/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Position Overview: Seeking a skilled Registered Nurse for a 12-week contract assignment. This position provides high-quality, patient-centered care in the home setting, focusing on Start of Care visits, ongoing assessments, and care coordination. The ideal candidate has strong OASIS and Homecare Homebase experience and is comfortable managing productivity expectations in a fast-paced environment. Schedule & Work Details: Location: Field-based role serving the Louisville Metro area Schedule: Monday through Friday, 8:00 AM to 4:30 PM Weekly Hour Guarantee: 32 hours Assignment Length: 12 weeks On-Call Requirements: One weekend per month Mileage Reimbursement: Yes, reimbursed between patient visits Pay Rate: $51 to $53 per hour Key Responsibilities: Provide skilled nursing care in accordance with physician orders and established plan of care Complete Start of Care visits, recertifications, and routine skilled nursing visits Perform comprehensive patient assessments and ongoing evaluations of patient condition Complete OASIS documentation accurately and within required timeframes Maintain productivity expectations of 30 points per week Coordinate care with interdisciplinary team members to ensure optimal patient outcomes Communicate effectively with physicians, patients, caregivers, and agency staff regarding patient status and care needs Deliver education to patients and families to support disease management and independence Ensure timely and compliant clinical documentation using Homecare Homebase (HCHB) Adhere to infection control practices and all regulatory guidelines Participate in on-call rotation as required Required Qualifications: Active Registered Nurse (RN) license in the applicable state Minimum of one year of home health experience Experience using Homecare Homebase (HCHB) required Strong knowledge of OASIS documentation and home health regulations Ability to work independently and manage a field-based schedule Reliable transportation and ability to travel within assigned territory Preferred Qualifications: Experience with IV therapy, wound care, and CHF management OASIS certification Strong clinical judgment and time management skills Excellent communication and patient education abilities About PRIDE Health PRIDE Health is the minority-owned healthcare recruitment division of Pride Global—an integrated human capital solutions and advisory firm.
