Job Title: Pharmacist
Department: Pharmacy
Reports To (Title): Director of Pharmacy
FLSA: Exempt
Summary:
Provides pharmaceutical services to long-term care and institutionalized patients in a manner that maximizes quality and patient safety. Participates in and oversees the dispensing of medication in accordance with federal and state regulations and Serviam Pharmacy policy.
Qualifications:
Education: Bachelor’s Degree in Pharmacy, Pharm D preferred.
Licenses/Certification: Current license in the state of practice.
Experience:
- Five years of experience as a licensed Pharmacist.
- Experience in leading or working within a cycle fill or compliance-focused Pharmacy, including Long Term Care, is a plus.
- Strong people, culture-building, and leadership qualities with a clear mission orientation.
Expectation of Hours: Hours of operation are 8:30 am-6:00 pm Monday through Friday. These hours may change to accommodate the needs of our customers.
Physical Demands/ Activity: Must be able to sit/stand for long periods of time. Must be able to lift 50 pounds. Management reserves the right to waive/modify any of these requirements.
Essential Functions:
- Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Makes recommendations for therapy changes to the physician and facility staff as appropriate.
- Maintains patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies).
- Ensures that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated, and discussed with appropriate members of the healthcare team.
- Collaborates with the healthcare team to provide clinical and technical support as needed.
- Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record.
- Implements policies and procedures for the safe procurement, storage, distribution and use of drugs and biologicals, as well as disposal of drugs.
- Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and Serviam Pharmacy policy.
- Informs the Director of Pharmacy of medication related errors.
- Assists facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering and disposing of drugs.
- Actively participates in Performance Improvement activities.
- Mental agility to understand our unique program and think of the best ways to scale it.
- Strong patient care orientation, self-motivation, and good interpersonal, communication, and problem-solving skills, and the ability to work effectively as a team player are required.
- Ability to demonstrate literacy in information systems and people operations.
- Performs other tasks assigned.
- Conducts job responsibilities in accordance with the standards set out in the Company’s policies and procedures, applicable federal and state laws, and applicable professional standards.
Knowledge/Skills/Abilities:
- Knowledge of pharmacy laws and federal and state regulations concerning long-term care.
- Ability to work cooperatively as a member of a team.
- Knowledge of pharmaceutical care practices for long-term care residents.
- Ability to maintain detailed and accurate records.
