AccentCare is seeking a dedicated Physical Therapist for our Home Health Weekends role to provide expert therapy to patients with health-related conditions to maintain, improve, or restore physical functioning, alleviate pain, and prevent physical dysfunction in patients. With a $20,000 bonus incentive, we are looking for professionals who are committed to enhancing patient quality of life in a home-based setting. This role requires traveling to patients' homes within assigned territories to deliver exceptional in-home therapeutic treatment, tailored to individual patient needs, adhering to highest standards of health care protocols.
Assess patient's condition during every visit and chart observations.
Perform evaluation tasks, including vital signs and medication review.
Administer physical therapy treatment as prescribed by a physician.
Instruct patients and families in proper home health care strategies and procedures.
Organize and prioritize patient load to ensure optimal scheduling.
Coordinate with occupational therapists, speech-language pathologists, nurses, and other health care professionals to optimize patient care.
Document patient care services by charting in patient and department records.
Proficiency in home health software
Strong interpersonal and communication skills
Ability to manage time effectively
Capable of handling physical workload
Expertise in developing patient care plans and carrying out interventions
