Job Summary
The Registered Nurse (RN) provides after-hours clinical support and triage services for assisted living communities. This role is responsible for assessing urgent resident situations, providing clinical guidance to healthcare staff, coordinating care with providers and families, documenting patient interactions, and supporting continuity of care. The RN serves as an extension of site clinical leadership, ensuring timely and appropriate responses to resident healthcare needs during non-business hours.
Essential Duties & Responsibilities
Triage incoming after-hours calls regarding urgent and emergent resident care situations.
Assess clinical concerns and provide appropriate nursing guidance and recommendations.
Coordinate care with hospitals, healthcare providers, families, and site leadership.
Support continuity of care and clinical decision-making for assisted living residents.
Document calls, assessments, interventions, and outcomes accurately in electronic medical records.
Monitor and respond to incoming calls throughout assigned shifts.
Communicate professionally and compassionately with residents, families, and healthcare staff.
Assist with emergency response and escalation procedures when necessary.
Maintain compliance with nursing standards, regulations, and organizational policies.
Build trust and effective working relationships with clinical teams and stakeholders.
Requirements
Minimum Requirements / Skills
Current Registered Nurse (RN) license in good standing.
Previous nursing experience in assisted living, long-term care, or related healthcare settings.
Knowledge of assisted living regulations, resident care processes, and clinical protocols.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal abilities.
Experience using electronic medical record (EMR) systems.
Strong organizational and documentation skills.
Ability to work independently in a remote environment.
Proficiency with technology and healthcare software systems.
Ability to manage multiple priorities in a fast-paced clinical setting.
