In this dual-function role, you will mentor staff, ensure clinical standards are met on shift, and support clients in developing skills for daily living, social independence, and vocational pursuits.If you are a passionate direct care professional looking to step into a leadership position while maintaining hands-on therapeutic engagement, this position offers the ideal opportunity to make a meaningful impact.Essential Duties & ResponsibilitiesShift Leadership & Operational Support

- Team Mentorship: Lead by example and mentor Life Skills Trainers in delivering high-quality, person-centered care and leading group activities.Clinical Liaison: Serve as the primary point of contact for clinical direction during shift; liaise with management and on-call leadership regarding resident needs, shift updates, and staffing coverage.Strategy Execution: Review, roll out, and communicate new behavioral plans, protocols, and strategies to ensure team-wide alignment.Data & Quality Control: Facilitate accurate, complete shift data collection and review necessary documentation for compliance with state, federal, and accreditation (CARF) standards.
- Behavioral Support: Implement behavioral management protocols, model pro-social behaviors, provide constructive feedback, and proactively address challenging behaviors with respect and empathy.Daily Living & Skill Building: Assist and guide residents with activities of daily living (ADLs), including medical/health needs, medication assistance, meal preparation, cognitive exercises, and recreational pursuits.Observation & Advocacy: Monitor changes in residents' behavioral, physical, or emotional health; advocate for resident needs and communicate observations promptly to the clinical team.Community & Environment: Maintain a safe, clean, home-like environment for residents; transport clients to medical appointments, group outings, and daily errands.Compliance Documentation: Maintain detailed logs, including goal sheets, vitals, seizure monitoring flow sheets, shift summaries, and incident reports while strictly adhering to resident confidentiality (HIPAA).
- Education: High School Diploma or equivalent required; Bachelor's degree in a healthcare, psychology, or human services field is strongly preferred.Physical Demands: Ability to lift up to 50lbs. Must have the ability to travel and/or operate a vehicle for up to 20% of the time spent working in this position.
